Pugliese is the adjective form of Puglia (Apulia), the long, sun-drenched region that forms the heel of the Italian boot. It is a name given to those who came from that region — either immigrants within Italy who moved northward and were identified by their origin, or families in diaspora communities abroad who were known as the people from Puglia.

Puglia is one of Italy's oldest inhabited regions. The Messapians and Peucetians lived here before the Greeks arrived and planted their colonies along the coast. The Romans called it Apulia and used its vast wheat fields to feed the empire. The Normans built their castles here, and Frederick II — Holy Roman Emperor and King of Sicily — made Puglia his heartland, building Castel del Monte and transforming the region into one of the great centres of medieval Mediterranean culture.

The region's agricultural wealth meant that it sent many emigrants, particularly during the great southern Italian emigration of 1880–1920. Pugliese families settled in New York, Chicago, Philadelphia, Buenos Aires, and São Paulo, bringing with them the traditions of focaccia barese, orecchiette pasta, and the ancient Messapian agricultural cycle.

In Italian-American communities, the Pugliese surname marks a family as southerners — part of the great mezzogiorno diaspora that shaped Italian-American culture more than any other regional group.
